Long term conditions include -
It is very important that patients suffering from a long term condition have annual reviews to ensure that their condition is being managed effectively and they have the correct support and management in place for their long term health. Please ring and make an appointment for your review or wait to receive your invitation letter.
Our nurses have been specially trained and offer a wide range of services. They can advise on health promotion, diets, female problems, contraception or many other problems that you may need help with. They are involved in the monitoring of patient chronic disease management i.e. diabetes, asthma, COPD and hypertension.
They also run our cervical smear service and any appointments for this test should be made with one of the nurses and not a doctor.
We like to protect our child population by doing their routine vaccinations ourselves. We hold vaccination sessions each month at the surgery. If you cannot attend a vaccination session, our practice nurses will be happy to carry out your child's vaccinations.
We offer the service of travel vaccinations and advice. We are one of a few designated 'Yellow Fever Vaccination Centres' in the Greater Manchester area. See the Travel Vaccinations section for more information.
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

* Please make sure that you make your appointment at least a month to 6 weeks in advance. You will need to complete a travel questionnaire prior to your appointment so that we can work out which vaccinations you will require.
It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
